1)  NEW MEMBER APPLICATION "ON-LINE" (Complete online and submit) You will be contacted as to when to pay your membership DUES using the link above. (QCTC Credit Card Dues Payments) All payments are via the above link.                Your signed FACILITIES WAIVER will also need to be mailed/delivered to the name and address listed at the top of the online application.

2) NEW MEMBER APPLICATION for "PRINTING" (Print PDF document and mail/deliver to the name and address listed on the form.) You will be contacted as to when to pay your membership DUES using the link above. (QCTC Credit Card Dues Payments) All payments are via the above link. Tennis Facilities Waiver also required to activate membership.

3) QC TENNIS FACILITIES WAIVER FORM (Required of all new members) Form cannot be submitted online.

Print Form (PDF document), SIGN and mail/deliver to address listed on membership forms/membership web page.

Club membership is not active until dues and all required paperwork is received by the Club.
